Nightmare is a boss consisting of two enemies, Genesis and Oblivion, fought in the Bellows. Their fight consists of two phases, the first against Genesis and the second against Oblivion. In Hard Mode, a third phase against both enemies is added.

Genesis fires three sets of bullets outward from its center, in a circular arrangement. The two halves of the circles mirror each other, with two large bullets opposite each other, and nine zig-zagging small bullets opposite each other. After a moment, the large bullets will slow to a stop and then explode into six small fire bullets that burst outward, in a hexagonal shape, as well as firing one medium-sized bullet each that are directed at the player.

Genesis hovers slightly back and forth at the top of the screen, spraying several medium-sized diamond bullets outward from its center directed in a triangular pattern. After a few moments, while still spraying diamonds, it will also fire off small fire bullets and long bullets. As this is occurring, block debris passes across the screen from either side.

Genesis fires off three consecutive spirals with several legs consisting of nine fairly densely-contained small bullets. The spirals are fired off in alternating clockwise and counter-clockwise directions. As this is occurring, several Temple clocks rain down from the top of the screen, descending at varying shallow angles left and right.

One side of the screen begins to shimmer, and a moment later, several long, icicle, and medium-sized blue bullets burst from outward. After a moment, their trajectory will pitch downward until they all exit through the bottom of the screen.

Genesis fires a line of six medium-sized bullets directed at the player and a burst of long, splinter-like bullets from its center (their angles of release also dependent on player position). The six medium-sized bullets initially travel straight outward, but then slow to a stop and, one-by-one, each begin to travel in an angle perpendicular to the initial line, alternating in direction left and right.

Phase 2 (Oblivion)

Oblivion appears, seemingly destroying Genesis. Four hands appear in a square formation and sixteen red, clumped bullets burst out from the center of the formation, their arrangement making the shape of an eight-pointed star.

Oblivion's hands hover into a line at the top of the screen. As they hover into place, they each fire off a few long, splinter-like bullets. After lining up, they will descend past the bottom of the screen one-by-one (in no particular order, it seems), leaving slightly curved trails of several medium-sized bullets in their wake.

Oblivion's hands form a diamond formation around it and spray two rows of clumped, medium-sized bullets each, in V-shaped patterns, from their eyes.
The hands then reposition, into a line, with the two hands on the outside slightly higher than the two inner hands. Each hand fires a series of medium-sized bullets from its eye, in a spiraling pattern achieving a ~225 degree angle from start to finish. The spirals are fired off in either a clockwise or counter-clockwise pattern, which alternates from hand to hand. As the formation is established and the spirals are released, Oblivion itself fires duplicates of its own eye in the direction of the player.
Once the spirals have finished firing off, the two inner hands descend quickly past the bottom of the screen, leaving small bullets in their wake. A moment after, the outer hands do the same.

Columns of hands form on the left and right sides of the screen. As they descend, hands opposite of each other take turns firing off lines of four small bullets.
While this is occurring, Oblivion rhythmically fires trios of medium-sized bullets in the direction of the player.

Oblivion hovers toward either the left or right of the screen and four of its hands zig-zag downward along the opposite side. Oblivion rapidly releases six-legged spirals from its mouth, the spirals alternating between clockwise and counter-clockwise directions. As they appear on-screen, each hand will fire off seven small bullets, directed at the player.

Hard Mode
Genesis and Oblivion fight against the player together in this phase. Oblivion's hands form slithering columns on the left and right sides of the screen for the remainder of the battle. During this phase, Genesis is unable to be damaged, and the player must focus on targeting Oblivion.

Additional hands of Oblivion appear in the center column of the screen while Genesis flies along the top of the screen dropping a flurry of diamond bullets upon the player. The diamonds fly upward briefly, then their trajectory pitches downward.

The columns of hands on the left and right squeeze closer to the center, limiting the player's space. Oblivion then fires three bursts of several bullets, many medium-sized, some small.

Oblivion releases a spiral of medium-sized bullets. Soon after, Genesis fires a laser at the player and pairs of bullets eject from its center in a fairly wide circular pattern.

Genesis warps around the screen three times. Each time, when it lands in position, it releases a burst of small and long bullets. In addition to this, it fires four medium-sized bullets in a downward column. They slow down quickly, and almost immediately upon stopping, explode into three small bullets directed at the player's position.

Notes
• Nightmare is a direct reference to the boss "Jenka's Nightmare" from the Cave Story fan-sequel of the same name, created initially by shmitz and revamped entirely later on by ZarroTsu. "Jenka's Nightmare" is a fight involving both Genesis and Oblivion, who appear as separate bosses earlier on in the game. The structure of the Nightmare fight in Star of Providence reflects the same progression: first, Genesis is fought, second Oblivion, and then (on Hard mode) they are fought together.
