"... You know, there have been others like you before. Two of them. They got pretty far down, deeper than anybody else before them. Maybe even... to the very end. Then... well. I don't know what happened to one of them. But the other is now the Overlord. Be careful, will ya."

Overlord is a massive mech suited with a great variety of visible weaponry, which is presumably attached to the top of the arena when you first encounter them by several large tubes. Overlord is the only boss of Sanctum, and is normally the first final boss that players encounter. In normal mode, their face is covered with a mask, which in hard mode, is removed to reveal an eye beneath inside the fleshy matter around it. Overlord's fight is mostly characterized by being heavily scripted and consistent.
For information about the ship with the same name as this boss, see Overlord (Ship).
Attack Patterns
Overlord has three distinct phases, each of which having its own health bar, each with less health than the last. Overlord's attack patterns are highly scripted and often loop upon themselves in the same sequences, although are independently subject to some variation depending on where the player is in the arena when any given attack is being used.
Phase 1
Phase 1 begins with Overlord being invisible, then appearing from the dark background, beginning the fight shortly after they're fully visible. Overlord's attacks loop in the same sequence until this phase's health is depleted.
Upon reaching 0 health in this phase, many portions of the mech are blasted off of Overlord as well as their tubing in a short cinematic, throwing them backward and onto the floor of the arena, which now appears to be old, torn up metal plating in the background. Overlord then transitions into phase 2.

Overlord opens up by shooting dense helix patterned fields of bullets from each of its turrets on its sides, which continue for the remainder of this attack. Overlord begins firing stars of bullets outward from near its center periodically, and from extra cannons mounted on its shoulders, begins periodically firing messy bursts of laser bullets toward the player, which alternate sides from where they're fired. After a short while, Overlord fires two laser bursts from both cannons simultaneously in quick succession, before moving onto the next attack.
Overlord fires rings of large bullets outward from cannons on its sides, and begins firing evenly spaced, two layered spirals of bullets from those same cannons, which continue for the rest of the attack. Overlord fires two streams of laser bullets from their shoulders each once, which after finishing firing, begin firing again at a faster rate, and instead three streams from each shoulder, resulting in the lasers firing between and outside where the lasers fired before.
Overlord moves to a location near the top of the arena and fires two oppositely angled spirals of lasers outward from that location for a short time, which appears as a pattern of nested diamonds. Overlord does this twice before moving onto the next attack.
Overlord will fire three series of thin spirals of bullets from the cannons on their sides, the first two being short and of different bullet speeds (second is slower than the first), and a third set of spirals which are slower and denser than the previous.
Overlord telegraphs a stream of bullets from its front cannons before firing a thick stream of large, fast fire bullets, and fires portions of rings of bullets outward from the cannons on its sides. Note that there's a safe space between the two streams of fire bullets where these rings won't reach if you're able to get there in time or are near there after dodging the previous attack.
Overlord fires a series of dense rings of bullets with evenly spaced gaps all the way around them from the cannons on their sides, alternating which side each time they fire, each ring accompanied by an evenly spaced, 4 layered burst of bullets directed toward the player. These rings and bursts have a great amount of speed difference within each instance of them, causing them to heavily overlap before they exit the arena.
Overlord moves back and forth across the top of the arena, quickly firing from each side cannon portions of rings of bullets outward that are often multi-layered. After this attack, Overlord loops back around to the first attack in this phase, and goes through the same sequence again.
Phase 2
After the short period of Overlord's immunity described before, phase 2 begins.
Upon reaching 0 health in this phase, more portions of Overlord's mech are blasted off, leaving them floating around the top of the arena with just their torso. The background rises again, to show intricate carvings in the wall which bear 4 large holes aligned vertically.

Overlord will fire out two dense spirals of laser bullets outward from its shoulders for a moment, and then fire the spirals again at a faster rotation rate for a shorter period of time. Overlord will then fire two rings of large bullets, jumping to another area in the arena.
Overlord will fire a series of large bullets toward the sides of the screen from their shoulders, which then upon hitting the side of the screen, explode into destructible bullets that travel slowly down toward the player, before jumping to another area in the arena.
Overlord will fire out two dens spirals of laser bullets outward from its shoulders for a moment, before dashing across the top of the arena while firing rings of large bullets as they move.
Overlord will dash somewhere at the top of the arena and begin firing a short stream of blood bullets from one of its shoulders, before firing two semicircles of blood bullets from that same shoulder toward the player, repeating this for the other shoulder.
Overlord dashes to various points at the top of the arena, telegraphing a laser they will fire directly downward from wherever they land each time, firing several neat, dense rings of large bullets as well as neat bursts to each side. After this attack, Overlord loops back around to the first attack in this phase, and goes through the same sequence again.
Phase 3
After another short period of invulnerability, Overlord will begin attacking again, floating from side to side in the arena until the phase ends.
Upon reaching 0 health in this phase, Overlord will stop attacking, and will slowly descend and then violently explode, ending the fight, generating the normal boss drop and allowing the player to exit and end the run from where they came, or if the player has all 4 seals, proceed to floor 6 and continue the run.

Video
Each cannon on Overlord's shoulders begins firing evenly spaced spirals of bullets rotating quickly in opposite directions, each of which have two layers of bullets per shot fired. The speed of fire from each cannon increases as this attack progresses, as well as the space between each shot fired getting progressively smaller.
Overlord will quickly fire walls of large bullets upward which terminate before they hit the top of the arena, which then generate bursts of blue bullets which travel downward, aiming slightly to the side the further to the side of the arena the player is in. As this attack progresses, Overlord fires these bullets faster, and with denser walls, causing the safe zone between bursts to become progressively smaller.
Overlord will fire several dense, neat legs of blue bullets in alternating spiral patterns from each cannon, with safe spaces half way through each wall of bullets generated. These walls generated have a small speed difference from each other when fired. This attack will continue indefinitely until the fight ends.
At the end of the fight, if this is the first time a player has defeated Overlord, they will drop the first seal to floor 6, and will unlock the methods by which the player will receive the other 3 seals (Renegade's offer, Bloke's offer, and Kleines' offer).
Notes
- Overlord is considered the first final boss of the game, as it's the furthest into the Facility a player can get on their first run and most new players are unlikely to encounter the other possibility so soon (note: big spoilers).
- Overlord is just one of a series of mechs that were presumably created for war.
